Most of us are drawn this field because we love children and we believe in the infinite possibility of childhood. But. Once we enter practice, there is often a feeling of being overwhelmed by all there is to know in order to be effective. This podcast was conceived to include you in our SpIRiTed conversations. These discussions have kept us engaged and had an unmeasurable impact on us as therapists. We want to share our regular clinical conversations with others, not knowing all the answers, but allowing each of us to have the space to think and put the pieces together. Tracy’s ability to make clinical reasoning and neuroscience accessible to all is incredibly powerful and worth sharing. ‘Show Notes can be found at www.spiritedconversationspodcast.com

Hosted on Acast. Tracy opens with a powerful quote from the Sensory Integration Theory and Praxis textbook that bridges sensory discrimination and executive functioning, and the conversation takes off from there.

We explore how Praxis doesn't arrive all at once but interacts with a child's changing developmental competence, from the six-month-old reaching for a toy, all the way to the basketball player executing a perfect shot.

In this episode, we dive deep into the clinical reasoning behind a case study of Cory's involving a 3.5-year-old boy. We do our best to link the experiences of this little boys daily challenges, such as dressing, social interactions, and emotional regulation, and unpack them through the lens of sensory integration and praxis.

We explore the vital difference between "high route" and "low route" brain processing of sensations and affect, and how we can use playful, relational interactions to bridge the gap. We also discuss the intersection of sensory integrative knowledge and Polyvagal Theory. With another aha moment in how the foundation of safety becomes a way we can facilitate sensory discrimination to emerge for motor planning and skill development.

Check out DFX's learning journeys to sign up for our learning journeys community and take any of the courses available to build your clinical reasoning skills --> https://dfxlearningjourneys.thinkific.com/

Full transcription of our episode is available on our website.

Timestamps

00:00 Introduction

01:30 Case Study: Functional challenges

02:52 Observations in Daycare

04:39 Therapeutic Approach and Techniques

07:04 Progress and Insights

09:15 Discussion on Sensory Integration and Praxis

19:17 Practical Applications and Tools

26:22 Playful Beginnings: Exploring Interaction

26:48 Progress and Social Development

29:23 Understanding Sensory Discrimination

32:28 Trusting Theories in Practice

33:19 Praxis in Action: Another Case Study

36:42 Reflecting on Clinical Observations

43:01 Developmental Pathways and Testing

Connect with us:

Instagram: https://www.instagram.com/spiritedconversations_ot/

Facebook: https://www.facebook.com/spiritedconversationsOT

YouTube: https://www.youtube.com/@spiritedconversations_OT

Website: https://www.spiritedconversationspodcast.com/

Loved this episode and want an easy cost free way to support us?

In this episode, we’re wrestling with something that comes up all the time in clinic: how do we explain the tricky clinical terms (like ocular control) to parents without losing them? We are continuing on the journey of trying to unpack language, talking about how we translate what we’re seeing in terms of ocular motor functions, vestibular contributions, and somatosensory discrimination into language that actually lands and makes sense for families.

We unpack how the vestibular system and sensory discrimination play into eye movements, how modulation bumps into these issues, and how neurodevelopmental factors shape what we’re observing.

This episode we discuss the importance of the somatosensory system in forming our sense of self through our relationships and how it shapes neurodevelopment. We explore how individuals who have not experienced secure attachment early in life may struggle with somatosensory amplification and somatization later on. We chat about the role of touch in creating a sense of safety and connection, and how sensory integrative processing can impact overall wellness. If you’re curious about the need to balance and respect boundaries while providing positive somatosensory experiences for individuals with neurodiversity this episode is for you. Hopefully, like we did, you get a deeper appreciation of the power of touch in building empathy, shared experiences, and a sense of belonging.

Check out DFX's learning journeys to build your clinical reasoning skills with Tracy --> https://dfxlearningjourneys.thinkific.com/

Timestamps

00:00

Importance of somatosensory system in attachment relationships and development

02:40

Impact of insecure attachment on somatosensory amplification and somatization

07:00

Neurodevelopment as a window into wellness and decline

10:00

Phantom limb experiences from missing relational experiences

12:30

Importance of sensory integrative processing and Dr. Ayres' work

16:00

Importance of touch in building empathy and connections

19:00

Enhancing somatosensory experiences in therapy

23:00

Balancing respect for individual boundaries and supporting adaptive function

26:00

Challenges of touch in vulnerable populations and the need for boundaries

30:00

Importance of psychoeducation and consent in touch interactions

Connect with us:

Instagram: https://www.instagram.com/spiritedconversations_ot/

Facebook: https://www.facebook.com/spiritedconversationsOT

YouTube: https://www.youtube.com/@spiritedconversations_OT

Website: https://www.spiritedconversationspodcast.com/

Loved this episode and want an easy cost free way to support us? When a child seems to be not registering the information from movement, how do we describe it? How do we talk about it?

[00:06:17]

Tracy: If you can help the sensory systems integrate with the vestibular system in a better way then that's likely to have improved impact on their function and development. The thing that changes all the time is the individual presentation of the behaviors that reflect the challenges with those underlying systems.

[00:16:55]

Tracy: If the person gets super activated by movement and then they might have a little bit of sympathetic activation. In the vestibular apparatus, as you move through space, you should have the fluidity and freedom of movement. But if you're not able to integrate and take in the details of space movement, that is more discrimination.

[00:21:36]

Cory: What's the difference between kids that have a hard time processing vestibular information and those that move all the time? Tracy: Focusing the conversation on that adaptive response is always a pivot point. And those kids are easier to treat because the treatment goes faster.

[00:31:04]

When things really come together for little developing kids, all of this circuitry just is sort of almost automatic. Sometimes we have to go a little bit slow to go fast. But if we remember to follow their lead, we can stick with them.

[00:37:50]

There is an indication that with prolonged Nystagmus that some of this treatment may have less efficacy. This one puts it centre stage.

Tracy, Cory, and Michelle work through what sensory discrimination actually is, how it differs from sensory modulation, and why Dr. Ayres was right to place it at the foundation of skilfulness and praxis. Along the way they get into haptic processing, the concept of affordances and action-perception coupling, and why a child who is working hard just to stay upright against gravity simply cannot prioritise sensory detail.

The key idea: sensory discrimination is not just a background process.

What does posture have to do with handwriting? More than you might think.

In this episode, Cory brings a composite case of a 12-year-old referred for handwriting difficulties, and what unfolds is a masterclass in reasoning from output back to the underlying sensory motor inputs that support it. The assessment findings — poor prone extension, disorganised ocular tracking, a striking tonic labyrinthine reflex response — point not to a handwriting problem, but to a nervous system that hasn't integrated vestibular processing deeply enough to give this young person the automatic postural stability he needs to sit, sustain, and write. Tracy uses a postural flow chart (downloadable in the show notes) to help organise those findings into a coherent clinical picture and walks through what treatment actually looks like for a child like this. We get into why fast linear vestibular input matters, what the ramp angle on a scooter board is actually doing neurologically, and why Dr. Ayres' approach to reflex integration remains just as relevant today as it was in the 1970s. The big idea in this episode: postural adaptation is not a skill you build through practice. It is an outcome of sensory integration. When it breaks down, you have to address the underpinning.

Timestamps:

00:09 Assessment overview — Beery VMI, COMPS, handwriting speed test, clinical observations

02:03 Tracy introduces the postural flow chart and why visual models support clinical reasoning

05:08 Tracy on sensory integration as the organisation of sensation for use

07:07 Introducing the sensory postural quadrad — vestibular, proprioceptive, visual, auditory

10:08 Tracy: why midline instability shows up clearly and what's lurking underneath it

12:41 Michelle: noticing he was already problem-solving his postural challenges before the assessment began

13:18 Cory on the effort visible in his body even before the tasks began

14:19 Tracy on affect, self-compassion, and why being sweet and cooperative doesn't help his postural control

17:04 Cory: walking through the full COMPS findings — prone extension, supine flexion, ocular tracking, saccadic movements, TLR

20:57 The tonic labyrinthine reflex test — flushing, eye flutter, near-fall, and what it tells us

22:52 Tracy: organising the findings using the postural chart — from basic vestibular processing through to handwriting output

29:32 Tracy: postural adaptation, centre of gravity, and the sagittal plane — a real-time explanation

32:34 Cory connecting the dots — vestibular integration, centre of gravity, and why the body recruits help

33:45 Tracy: the definition of postural adaptation and why it's one of the primary products of sensory integration

37:25 Treatment planning — starting with fast linear vestibular input and why the ramp angle matters

38:51 Tracy: scooter board ramps, platform swings, lycra swings — how to inch toward vestibular activation safely

39:48 Michelle: why we sometimes need to dampen the overwhelm before we can build through it

43:15 The role of auditory input as a partner to vestibular and proprioceptive processing in treatment

49:22 Why automatic postural control is the goal, not conscious compensation

49:56 Tracy: it doesn't get better with practice; it gets better when you address the underpinning

50:24 The origins of the postural flow chart — Anne Grady, Children's Hospital Denver

If this episode resonated with you, please share it with a colleague and take a moment to leave a review — it genuinely helps more people find us.

Check out DFX's Learning Journeys to build your clinical reasoning skills: https://dfxlearningjourneys.thinkific.com/

Connect with us: Instagram: https://www.instagram.com/spiritedconversations_ot/ Facebook: https://www.facebook.com/spiritedconversationsOT YouTube: https://www.youtube.com/@spiritedconversations_OT Website: https://www.spiritedconversationspodcast.com/

Loved this episode and want an easy cost-free way to support us?
