Today’s guest is Dr. Vaninder Dhillon, an assistant professor of Otolaryngology - Head and Neck Surgery at Johns Hopkins University School of Medicine, practicing in Bethesda, Maryland. She is dual fellowship trained with expertise in thyroid and parathyroid surgery and radiofrequecy ablation for benign and malignant lesions, as well as laryngology which includes voice and swallowing disorders. She has a clinical interest in laryngeal outcomes after thyroid and parathyroid surgery, as well as radiofrequency ablation.

Becki underwent percutaneous laser ablation treatment to shrink her largest thyroid nodule. Newly available in the United States, laser ablation shrinks thyroid nodules without surgery. Laser ablation for thyroid nodules is a newly available treatment option  in the United States. It is similar to RFA in that it uses a tiny probe  to ablate and shrink a thyroid nodule. Today's guest, Dr. Stefano  Spiezia, is an expert on this technology and recently traveled to the US  from Naples, Italy to conduct physician trainings across the country.  Today, we'll learn more about the laser procedure and how it helps patients save their thyroid. 

Stefano Spiezia, M.D. Ph.D. is Italian board certified in general, emergency and endocrine surgery and in general diagnostic and interventional ultrasound. He received fellowship training in endocrine  surgery in the University of Naples Italy, and is Adjunct Professor in  Interventional Ultrasound techniques of Thyroid Pathologies in the  University of Turin, Italy. Dr. Spiezia currently serves as Chief of  Ultrasound Guided and Endocrine Surgery in “The sea hospital” in Naples,  Italy. His areas of clinical interest are endocrine, oncologic and  robotic assisted thyroid surgery, ultrasound guided procedures and  minimally invasive techniques for thyroid pathologies.

In today's video, we're joined by Dr. Agustin Andrade of Miami, Florida, who's been doing PEI (percutaneous ethanol injection) for thyroid cysts with excellent results for over 2 decades. For many years, the standard of care for managing thyroid cysts was surgery. Dr. Andrade wants  doctors to know there is an inexpensive and effective alternative  treatment option that preserves the thyroid gland.  

Dr. Andrade is Chief of the Endocrinology Department at Mount Sinai  Medical Center in Miami, Florida and he has been in practice there since  1997. He holds the unique distinction of offering his patients not one, but three nonsurgical thermal ablation techniques for thyroid nodules! Providing PEI for thyroid cysts, and thyroid RFA and laser ablation for  solid nodules, Dr. Andrade can choose the treatment option best suited  to a patient's individual needs.

Dr. Catherine Sinclair is no stranger to being a ground breaker. In 2019, she  performed the first thyroid RFA in the state of New York, at Mount Sinai  Hospital. But by 2021, she found herself relocating to her native  country of Australia. Dr. Showmaker is a thyroid focused ENT physician based out of Merriam,  Kansas in the Kansas City Metro area. I had the pleasure of meeting Dr.  Showmaker earlier this year in Fort Lauderdale at an RFA physician  training program. Since then, he has implemented this procedure into his  practice and is helping save thyroids from surgery. Today's guest is not only saving thyroids from surgery, but she's also  training the physicians of tomorrow. Dr. Julia Noel is an assistant  professor of otolaryngology/head and neck surgery at Stanford Medicine. Together, with her colleague Dr. Lisa Orloff, they brought thyroid RFA to this prestigious institution, and they've been saving thyroids for  well over a year now. One thing I hear on the regular is how sick and tired thyroid patients are of being sick and tired after thyroid surgery. Dr. Sean Nikravan was  familiar with this common complaint from his patients. Eager to offer  them a better way, he adopted thyroid RFA as a treatment option for  thyroid nodules, and his patients couldn't be happier. Most physicians who offer thyroid RFA are endocrinologists, ENTs,  interventional radiologists, and surgeons. But cytopathologist Yasmin  Elshenawy saw the value that thyroid RFA could bring her patients. She  researched and sought out training in this innovative modality on her  own. In doing so, she not only became the first RFA provider in the  state of Tennessee, but also the first cytopathologist in the United  States to do so. Dr. Elshenawy is paving the way for other cytopathologists to join the ever growing interdisciplinary team of RFA  providers in the US.

Today, I'm catching up with Dr. Kathleen Hands of San Antonio, Texas for the second time. As the first endocrinologist in the country to become ECNU certified (Endocrine Certification in Neck Ultrasound), she possesses tremendous skill in use of the technology to accurately perform  ultrasound-guided biopsies on thyroid nodules. This is a vital skill for a thyroid-focused physician, and has translated to her adoption of thyroid RFA. Since late 2019, Dr. Hands has been saving thyroids through this nonsurgical procedure; not only by treating her patients, but also through training fellow doctors. In speaking with Dr. Hands, its clear that she is passionate about this topic. She feels strongly that RFA is the best option for preserving many patients' quality of life.
